Fees: Lactation & Feeding Services (breast and bottle feeding only, no solids)

Evaluations/Initial Appointment

• Initial evaluation with written report: $550.00

• Initial evaluation with 1 page treatment plan: $425.00

• Initial evaluation with verbal report only: $350.00

• New client with an outside evaluation: $130.00 + Treatment Session Fee

  • This fee is if you are a new client with an outside feeding evaluation report. The fee goes towards the time the therapist spends with new client intake, chart review, therapy planning and updated goals.

• Please add a $50 travel fee if seen outside of the clinic (e.g. home or daycare)

• Please allow 60-90 minutes for this appointment

 

Follow-up Sessions:

• $200.00 per 60-minute session

  • 50 minutes of treatment and 10 minutes for documentation and planning.

• $170.00 per 45-minute session

  • 35 minutes of treatment and 10 minutes of documentation and planning

• $130.00 per 30-minute session

  • 25 minutes of treatment and 5 minutes of documentation and planning

• If sessions extend beyond 60 minutes, additional time will be prorated at be $50 per 15 minutes increments.

• Please add $25 travel fee per session, if seen outside of the clinic (e.g. home or daycare)

 

Extras: Meetings, Phone Calls, Observations, Consultations, additional reports, etc.:

$200.00/60 mins, $170/45 mins, $130/30 mins.

• If extended beyond 60 minutes, additional time will be prorated at be $50 per 15 minutes increments.

• Please add a $50 travel fee if seen outside of the clinic (e.g. home or daycare)
