INSURANCE & FEES
Private Pay: Assessment and Treatment are available. You can also contact your insurance company to inquire about using out-of-network benefits (see below for more information).

Initial Session/Assessment:

$350.00: This includes a full assessment of your child with a written a treatment plan/home program. This does not include a detailed written report, but goals/treatment plan only.

$500.00 full assessment with detailed report. You must choose this option if you need a report with detailed information to share with other providers (e.g. ENT, Pediatric Dentist, Gastroenterologist, School SLP, etc).

Therapy Sessions:

$180.00 per 60-minute session
-
50 minutes of treatment and 10 minutes for documentation and planning.

$150.00 per 45-minute session

35 minutes of treatment and 10 minutes of documentation and planning

$110.00 per 30-minute session

25 minutes of treatment and 5 minutes of documentation and planning

If sessions extend beyond 60 minutes, additional time will be prorated at be $30 per 15 minutes increments.

Meetings, Observations, Consultations, additional reports, etc: $180.00/60 mins, $150/45 mins, $110/30 mins.

This includes formal meetings (e.g. IEP meetings, IEP development, meeting with other specialists, consultations with other professionals, etc.

Consultations without a written report or treatment plan (to gain information)

If extended beyond 60 minutes, additional time will be prorated at be $30 per 15 minutes increments.

Early Intervention Services: we are providers with Golden Gate Regional Center for early intervention services.
Insurance: we are currently in-network providers with Blue Shield of California and Kaiser Permanente.

Blue Shield of CA: we should be covered under most PPO plans, but we can verify prior to assessment or treatment.

Please do not confuse Blue Shield of CA for Blue Cross/Blue Shield or Anthem Blue Cross. They are different companies.

Kaiser: Kaiser Permanente has their own internal assessment and referral system. You must first be assessed at Kaiser and referred out for treatment.
· Other insurance: you may be able to receive reimbursement from your insurance company for out-of-network providers. This is the responsibility of the family to arrange this with your insurance provider. We can provide you with a "superbill", which includes diagnostic and procedure codes that can be submitted to your insurance company. We ask that families act as the liaison for any direct communication with their insurance companies.
